Student View - Needs Assessment Panel

 

This is where you enter the Needs Assessment Data for each student. MEOPs have a Needs Assessment form to collect this information. Every site does a Needs Assessment for every student annually.

 

There are 14 fields on the Needs Assessment Panel. Except for the Grade field they are all check box fields. A check means Yes there is a need. If there is no need leave the field blank.

 

Be sure that you have the correct school history line highlighted in the grid below.

 

Grade field - Most of the time you will have entered this when doing the enrollment on the School History Panel, but you can change it here if need be.

 

Education Interruption - student who in the preceding 12 months changed school buildings or missed significant amount of school time (10 days or more) during the regular school year (usually defined as September through June) due to the student's family's lifestyle.

The above should read "School buildings"? Consortium approved 3/1/2006 - effective 9/1/2006

 

Special Education -include only students with an IEP

 

Parent(s) Involvement - parent(s) who are not involved in child’s Education by: i.e. attending school Activities and/or encouraging the child in his/her educational program]

 

Health/Nutrition - children who have health/dental or ongoing nutritional needs

 

Grades - children who have low academic grades, ie

 

  • K-8 - any student scoring C, S- or an Unhappy Face, 75 or below, any marking periond in any core subject (English Language Arts, Math, Science, Social Studies and Foreign Language)

 

  • 9-12 - Student scoring 75 or below in any marking period in any credit bearing class

 

Retained - student is repeating the same grade as last year. Students going from pre-First to First or DK to K are considered to be retained

 

Attendance - student’s absences exceed his/her school’s policy

 

Credit Deficient - student in grades 9-12 who have failed a course during their high school career and have not yet made it up or students who do not have enough credits to graduate with their cohort

 

State Assessments - student has failed at least one state assessment

 

Below modal grade - student is not age/grade appropriate

 

LEP - student limited in English proficiency, for in schoool student, based on school district assessment (Commissioner Regulations Part 154). For out of school and preschool students as indicated on COE.

 

Suggestion from East Bloomfield -

 

Student Data ~ Needs Assessment Screen:

PLEASE do not continue to have Credit Deficient historical.

This is creating a problem with record keeping.

Examples:

1 - a student becomes Credit Deficient in 11th grade, and suddenly he is

Credit Deficient in EVERY year (from P0...all the way thru 11th grade)

2 - a student becomes credit deficient in 9th grade, (drops out) then

becomes a DO....it still records him as being Credit Deficient as a DO.

 

Also, with LEP: a student could be LEP when they are censused on the

COE, but can drop the LEP status as they become proficient with their

English skills.....there is no way to determine WHEN the student was

no longer LEP, as it is "erased" from ALL years. This could also be a

problem when an audit occurs, as they may have been LEP during the year

in question, but have since dropped their LEP status.
